[Coco] Setting up the /IH descriptor

I've created a new boot disk using the latest version (built from source) with the /IH descriptor.  I'm getting Error 214 - No permission when I try to access a drive.

I've tried dmode /IH STP=ff which is an os9 disk but any disk I try to access gives me the same error.  The permissions are correct on the IH descriptor when I made the boot floppy.  There must be more to the config.
