Phil,

That is very unlikely. More likely is that you don't have the required descriptor for /D0 or /D2.

Use the command dmode to determine the size and type of drive permitted. You want to see:
dmode /d0
lots of data + cyl=28 sid=02
If you see cyl=23 and sid=1 then OS-9 thinks the drive will be a 35 track single sided drive. Change 
it with dmode.
dmode /d0 cyl=28 sid=2
