[Coco] How to make PMODE 4 CSM video files

Hi All, 

For anyone who wants to make your own CoCo CSM videos with artifact colour (or black and white only) to be played back with Ed Snider's SDC Media Player from the CoCoSDC. These videos can be played back on a CoCo 1 with 64k and 6309.
