As well as the 26-3129, it is also a +5V only controller. 

As I recall this was the controller used on the Mini Disk systems.

The FD-500 and it's immediate predecessor the 26-3029 are 5 volt 
controllers. The 26-3022 is the only controller that requires 12 volts.
