If all else fails, try cleaning the drives.  I found that a dirty drive 
will cause a lot of really weird things.  If you don't have one, 5.25" 
drive cleaners are available on eBay.  I've copied thousands of Coco 
diskettes to my Cloud-9 hard drive, and every once in awhile I'll run 
into an old diskette which has the media scaling off.  Often this disk 
will sound raspy when spinning in the drive.  It often results in a 
drive failure where doing a DSKINI will appear to work during the first 
pass, but will fail in the verification second pass.  Cleaning the drive 
will take care of this problem almost immediately.  Even if this isn't 
your problem, having a drive cleaning diskette and cleaner fluid is 
highly recommended.  Old diskettes do degrade.
