Hmmm.... I just did it again here and all was well. You should never have to
chd or anything, the whole time at the /dd prompt is how I did it. dsave
gave me an error 214 at some point but it doesnt hurt or stop the copy.

Anyway, here is the disk, all fixed for dskini.
There are a couple versions of dskini out there but I used the one at this
link.

DSKINI /T40 /D B: LSL_FIXED.OS9
