You are right. Support for headerless .os9 disk format was dropped in MAME
0.163. Tim Lindner added support for JVC header in MAME 0.187. So in MAME
0.186, I get an "Invalid disk image" error when trying to open a SQ1 disk
with a JVC header. SQ1 and SQ2 with a JCV header lock up in MAME 0.187 and
up.
Support for headerless OS-9 disk images was added back in MAME 0.189 (no
need to rename the disk to .os9) but I get the same result with a headerless
SQ1 disk. However, if I hit enter before the disk stop spinning (you can now
hear it in the emulator), I get to the login screen and the first room. But
as soon as I go into another room the game locks up.
I'm surprised that you can open disks with a JVC header with your version of
MAME as it was not implemented yet. Did you apply any custom patches to your
version?
