It's been broken from my perspective for a long time. Biggest issue is 
orphaned posts from otherwise threaded responses.

I'm no internet expert but I'm pretty sure it is a combination of how 
people choose to respond, and your mail client. I'm using Thunderbird 
and it's relatively easy to confuse it if my few dozen mailing list 
subscriptions are anything to go by. :(
