I've triea many different cable configurations, and nothing seems to work. (The 5.25" drive does NOT have a selector to choose between 0 and 1, and I don't see a terminating resistor.)

Bill,
The 5.25" drives don't need twists. The 3.5" drives may depending on which
numbers you want to assign to them.
A single 3.5" drive will act as single sided drives 1 and 3 without a twist in
the cable. I will also act as a double sided drive with NitrOS-9 as drive #1.
If you insist on having four physical drives, you will not be able to use them
as double sided drives on the same controller. You should be able to have #0 -
5.25", #1 - 3.5", #2 - 5.25", #3 - 3.5" without any cable twists as single sided
drives.
