I have a nephew that works at a "custom" metal shop near me. He does laser cutting on all kinds of racks, enclosures etc. He said he may be able to get small runs of stuff done. I've offered this before but no one seemed interested.
I can have him check on getting some cases done J&M style (long & short). These are nice 2 part cases with no major cutting or bending. I can take measurements off my J&M controller and give him the specs and see what he comes up with.
I can take pics of the case so you can see how they're made. I would say get blank cases and do your own drilling for screw hole and spacers. Keeps cost down.

Has anyone worked out one that can be laser cut and assembled?   There are some  
vendors who do that at pretty reasonable cost.  I have yet to do this with  
anything too many projects right now but at some point I am going to try it.

I was machining cases for Roger when he needed them.  I still have some  
extra cases available, these are the half cases used for program packs.   
I will get some photo's later today.  The biggest issue is these are  
machined and take a fair amount of time to make.
